I’m taking a little survey. When one is evaluating search results, is that called a relevance test or a relevancy test?

It doesn’t matter. Just call it a “relevance test” and move forward. 🙂

Around here we call it a “Test to see if the user is really getting what they want to see test” We find that this adds clarity that is missing from the terms “Relevance” and “Relevancy”.
I say both are fine. Depends on how fond of using the letter “Y” you are. I suppose you could say that if the subject of your test is relevance, then it is a relevance test, and if the subject of the test is a particular thing, and you want to know its relevance in relation to various other things, that would be a relevancy test of said thing. I’d say they’re really the same, though, because you could kind of reverse that argument.
